4-Step Checklist to Get on the Ballot

  1. Find Your Precinct: Go to http://mi.gov/vote to find your precinct number.
  2. Complete the Affidavit: Fill out the Affidavit of Identity.
    • Note: Ensure your name is written exactly as you want it on the ballot.
  3. Get it Notarized: Your signature must be witnessed by a Notary Public.
    • Tip: Free notarization is available at the Ingham County Clerk's office.
  4. File the Form: Submit the notarized form to your county, city, or township clerk by May 5, 2026 (4:00 pm).
